VOLUNTEER
Start here
Volunteer clearance is required for adults who have frequent or prolonged interaction with students, including but not limited to field-trip or overnight chaperones, coaches and assistant coaches, and regular weekly classroom or campus volunteers.

Connect
Your event sponsor, teacher, or coach directs you to the volunteer coordinator.

Complete Live Scan
Begin with Live Scan. It is the longest step and can take 1-3 weeks.

Collect your documents
Complete the required training, forms, and TB evaluation. Drivers also gather their DMV record and insurance.

Submit once
Scan all completed documents into one packet and email it to Volunteer@lsak12.com.
What to include in your packet
All Volunteers
- One time:
- – Live Scan background check (BCIA 8016).
- Each year:
- – BCIA 9018 background-check consent.
- – Mandated Reporter certificates.
- – Signed Volunteer Commitment form.
- Every four years:
- – TB risk assessment or other medical clearance.
- – Copy of a current photo ID.
Additional Requirements for Drivers
- One time:
- – Copy of a valid driver’s license.
- Every 6-12 months:
- – Update insurance information upon policy renewal.
- Every year:
- – Renew the completed Vehicle Information Sheet.
- – Official DMV Driver History Record.
